This form will not be produced under these conditions:
- If the Total Revenue on Line 10 of forms 05-158A or 05-169 is less than $1,110,000 threshold
- If the annualized of revenue is below the "No Tax Due" threshold
- If return is qualified for EFT (Electronic Fund Transfer)
Form 05-170 can be forced by using Texas > General > Basic data > Force Print payment Form 05-170 field with code {Y}
